Chronic Care Management, Certificate (Graduate)

Format: Online

Program Description:

Colorado Christian University offers two graduate certificates for RNs with a BSN degree (or higher) to help them update and enhance specific competencies. These theory and clinical practice certificates can help RNs advance their career and augment their current nursing practice.

Each certificate requires four courses, 325 clinical hours, and 30 weeks (7-8 months) to complete. The coursework addresses evidence-based practice and advanced nursing concepts within a biblical framework, focusing on servant leadership within professional practice.

The Chronic Care Management certificate covers models for quality outcomes with adult and geriatric patients, expert care for these populations, and includes both client-focused and population-focused clinical care management practicums. Students have the option to use graduate certificate courses completed at a B or higher grade within a degree emphasis if they apply and are accepted within five years into the Master of Science in Nursing program.
